Purpose Request to access municipal records or to correct any personal information held by the municipality, in accordance with MFIPPA.

This application allows individuals to obtain municipal records or view the personal information a municipality holds about them under the Municipal Freedom of Information and Protection of Privacy Act. It also provides the right to request corrections if any of that personal information is inaccurate or incomplete.

Fee

As per the Fees and Charges By-law (Clerks and Legal Schedule) a $5 fee applies to each application.

How to Submit a Request

Completed forms can be submitted:

  • by mail: Clerk's Office, Town of Midland, 575 Dominion Avenue, Midland ON, L4R 1R2
  • in person: 575 Dominion Avenue (Monday–Friday, 8:30am–4:30pm)
